Release of Records Forms –

Parents, please complete the appropriate form when requesting that your child’s teacher(s) complete any kind of survey, query, information request from anyone outside the Douglas County School District.  This applies to doctors, psychologists, schools, etc.  You will also complete this to have your child’s records transferred to a school outside of the District, or any private school.  Call our Registrar at 720.531.3309 should you have questions regarding this.

Policy on Student Education Records  – this is the policy regarding releasing student records.

Release of Records to Third Party – This is to request student records be sent from us to a third party.

Request to Inspect and Review Student Education Records – A parent or guardian will complete this form if they wish to review their child’s records and/or receive copies of said records.

Request for Release of Student Information – Parents will complete this form to transfer their child’s records TO our school.
