Notarized POR Form – if you live with a relative or friend, you’ll provide this form along with their proof of residence as listed on the Lottery Application

Child Statement of Health  Required annually for Preschoolers and Kindergarten students.  *Please note:  The Child Statement of Health is not due until 2 weeks before your child begins school.  We just want you to have plenty of time to visit your physician to get this completed.
